PRIVACY STATEMENT

Power4Bones (P4B) is an educational project created by the Registered Dietitians at Dairy Farmers of Canada. P4B is designed to help Grade 5 children build stronger bones through physical activity and proper nutrition.

The P4B Web site collects as little personal information as possible from all users, including teachers, students and parents. While the Web site includes highly personalized elements, we have worked hard to protect the privacy of all participants.

Dairy Farmers of Canada/Alberta Milk will never share any information collected with any type of third parties. Here is a list of information that is collected, and how we use it.

Information collected about all student participants:

• First name
• Age
• Gender
• Usage statistics, such as scores, how many times each user played a game, browser type, etc.

Information collected about all participating classes:

• School name
• City
• Teacher's name
• Principal's name
• Local Hospital name
• Local shopping mall name

Information collected about participating teachers and schools:

• Teacher name
• School name
• Number of students
• Principal name
• Contact information
• Statistical information about P4B participation

Information collected about students who complete participation surveys:

• Gender
• Nutritional information
• Physical activity information
• Information retention questions
• Behavioral questions

Information collected about teachers who complete participation surveys:

• School identification code
• Answers to survey questions
• Opinions about the quality of P4B
• Opinions about P4B usage by teachers and students

Information collected about people who use the "Contact Power4Bones" form:

• Name
• Email address
• Question
